To obtain EAPA assistance from Community Support and Outreach Services you must live on the Central Coast NSW.
Complete the following form and submit. Once submitted, you will then receive an email from the EAPA team - you will need to reply to this email with your required documents before you can book an appointment.
Documents required are:
• Current energy bill(s) including both sides of all pages (reminder or disconnection notices are not acceptable)
• Photo ID, stating your name and address as shown on your bill
Once received one of our friendly team members will be in contact with you.
